Acceptable Proof of Identification and Residency

Identification:

1. NYS Driver’s License or ID card issued by NYS which contains a photograph and    information such as a name, date of birth, sex, height, eye color, and address.
   

2. ID card issued by federal, state, or local government agencies or entities provided it contains a photograph or information such as name, date of birth, sex, height, eye color, and address.
   

3. School ID card with a photograph.
   

Residency: (Date must be current within the last THREE months)

1. Utility Bill (i.e. Con Edison)
   

2. Phone Bill (landline Only)
   

3. Rent Receipt
   

4. Cable Bill

NO CREDIT CARD OR CELL PHONE BILLS!
